كتابة اسم المريض بالكامل -اذا وجد المريض مشكلة في كتابة اسمه بالكامل لضمان خصوصية حالته بامكانه كتابة اسمه الاول فقط ولكن عند رسال حالته ليطلع عليها الدكتور زياد وفريقه الطبي-مركز الاردن للعظام والمفاصل والعمود الفقري يجب كتابة الاسم بالكامل
